  • Are you ready to get started? Below are the necessary chiropractic forms required to receive care at our office for your initial consultation. This information is routine, required for our records, and mandated by state and federal statutes for the safety of you and our staff. It is not required that you fill out these forms prior to your first visit. However, it does make the process faster and more efficient upon arrival for that initial appointment. If you have any questions about what’s on the forms or any other questions about our business, please don’t hesitate to call or contact our staff during office hours.

  • Appointments & Cancellation

    You can schedule appointments via the internet or over the phone. Please provide at least 24 hours of notification for appointment cancellations to avoid a fee.

  • What to Expect at your Initial Visit

    Receiving chiropractic care may seem like a daunting endeavor if you’ve never been to the chiropractor. At our facility, we take pride in making everything relaxing for our patients, which helps treatments to be that much more effective. Here’s what you can expect from that initial visit with Dr. William Byrkit.

    • You’ll be greeted by our friendly receptionist that will take care of any paperwork and clerical items with you.
    • You’ll be seen as close to your appointment time as possible to limit the time you have to wait. If you’re early, please have a seat in our comfortable waiting area.
    • You’ll be called by a chiropractic assistant to your treatment area.
    • Either a chiropractic assistant or Dr. Byrkit will go over your patient history and specific symptoms with you.
    • Dr. Byrkit will then conduct your chiropractic exam and x-rays the same day.  The next appointment a consultation and any adjustments will be made.
